  1. You are a private practice dietitian, and today you are seeing your patient, Rebecca. She is a distance athlete at the local community college, and on average she runs 5 miles each day. She is 22 years old, is 56 and weighs 110#. She understands that it is important to eat three meals a day in order to fuel her athletic performance, but states she doesnt feel like she always does the best that she can and used to at practice.

You do a 24-hour dietary recall with her. She tells you that yesterday she ate half a bagel with cream cheese for breakfast, a peanut butter and jelly sandwich for lunch, an apple and string cheese for a snack, and one chicken taco with a side of rice, about cup, for dinner. She does stay hydrated and drinks usually 2-3 liters of water each day and sometimes will drink a sports drink.

You ask her about her weight history and she tells you that her weight has been trending down for the last year and she noticed this started after she joined the track team.

Based on this information, identify her key nutrition-related problem and write a PES statement.

2. You are the inpatient dietitian in a hospital when you go to visit your patient Shelby who has been admitted due to having chest pains. She is okay otherwise but needs to be monitored by the medical staff. She has been complaining of being hungry throughout the day and says that her trays are not enough food to settle her appetite. She is a 46-year-old female who is 53 and weighs 210#. You go to her room and perform a 24-hour recall on her. She tells you that yesterdays intake was very normal for her. She ate two breakfast sandwiches with eggs, bacon, and cheese, as well as a hash brown for breakfast.For lunch, she had sweet and sour chicken and fried rice. For a snack she had grapes and milk. She tells you that her dinner consisted of pasta, garlic bread, and a salad. She ended of her day with a bag of chips, which she normally does at home in front of the television.

She tells you she has been advised to change her diet in the past, but its hard because this is how her family eats at home and when she tries to make healthy choices they do not like them and will still eat this way. So she says its easier for to eat what her family eats.

Based on this information, identify her key nutrition-related problem and write a PES statement.

3. You are an outpatient dietitian seeing your patient Steven. Steven is a 72-year-old man who lives by himself. He recently had a stroke about a two months ago, so he actually has issues standing for too long and managing around the house, especially since he feels his right arm isnt like it used to be. He lives alone because his wife passed away 2 years ago. His daughter lives about 30 minutes away and usually stops in to check on him once/week. He doesnt cook as much after the stroke because he says its too hard for him to do much physical work. Instead, he usually walks the block to the neighborhood fast food restaurant almost daily. Most days this is his only meal.

He is 62 and weighs 145#. He states he feels like his weight has gone down since his stroke. He doesnt own a scale but his clothes are getting bigger.
