– Module 3 Case Study Discussion Assignment. Mr. ABC. Today you're off from the clinic and are covering a shift in the emergency department. You go to see Mr. ABC, a 34-year-old Hispanic male, who presented with a complaint of severe dyspnea. He has been experiencing dyspnea for two weeks that has slowly, but progressively gotten worse. He was brought back immediately from triage because he had a respiratory rate of 40, an O2 sat of 88%, and a temperature of 101. His O2 sat has improved to 92% on four liters of oxygen via nasal cannula. Review of symptoms is positive for fevers, chills, weight loss, malaise/fatigue, diaphoresis, cough, dyspnea, weakness, swelling in the lower extremities, feeling of abdominal fullness, and unintentional weight loss. He denies any other symptoms. His past medical history is significant for Asthma. His past surgical history is negative. His social history. He's a single male and has sexual relations with both men and women. He does not deny smoking. He has a history of IV drug use but has not used any IV drugs for five years, and has a history of a heavy alcohol use. But he hasn't drank for two years. Physical exam. Vital signs. His temperature is 101. Heart rate is 120. Respiratory rate is 38. Blood pressure's 110/60. And his O2 sat is 92% on four liters of oxygen. He's alert and oriented times three and is thin. He has a normocephalic and atraumatic head. Pupils are equal, round and reactive to light. There's no nystagmus or scleral icterus noted. He does have white patches on the tongue and buccal mucosa bilaterally. The neck is supple. There's no JVD. There's no tracheal deviation. There's no thyromegaly or thyroid nodules noted. He has a normal rate. A normal S1, S2 is present, without an S3, S4, gallop, friction rub or murmur. There is 1+ pitting edema in the lower extremities. The brachial, radial, dorsalis pedis and posterior tibial pulses are 2+/4+ bilaterally length. He has dyspneic. His respiratory rate, respirations however are regular and even. Lungs do have rales in all lung fields. His abdomen is round and distended but soft. Bowel sounds are active. There are no masses noted. No CVA tenderness. He does have some hepatosplenomegaly present with some shifting dullness. There's no rigidity, rebound or guarding. He has widely spread lymphadenopathy.Cranial nerves II-XII are intact. The skin is warm, dry and intact. A line of vesicles on the left thorax that extends from the anterior to posterior thorax is present. Mood and affect are normal. Calm and cooperative behavior. And his judgment is intact. Upon completing the history and physical Mr. ABC ask you, “What is wrong with me? Why am I sick? What do I need to do to get better?” Your objective is to answer Mr. ABC's questions. You are to record yourself answering Mr. ABC's questions and post the video to the discussion board. To answer his questions completely, you must integrate the answers to the questions below into your response to Mr. ABC. Your recorded response should demonstrate that you are speaking with Mr. ABC as though you were in the room with him. Question one. Based on the current information available, what diagnoses do you believe Mr. ABC has? While your diagnoses still need to be confirmed, you have enough information to know what is wrong with Mr. ABC. What labs and/or diagnostic tests do you believe are needed to confirm your diagnoses? Number three. Describe the pathogenesis which explains the patient's condition? And number four, what education is important for Mr. ABC to receive now? Assignment instructions. This week's assignment is a graded assignment and required. You are to record yourself answering Mr. ABC's questions. The recording requirements are as follows. You must face the camera directly during the entire video. The camera must be on your face as you are talking. The webcam will serve as a proxy for the patient, hence you should talk directly into the webcam as though it is the patient. This video is of you talking to the patient and should show you from the shoulders up. You must present a professional demeanor in the video and use terminology that is appropriate for the target audience. You must be professionally dressed. You may have note cards you refer to, but you should be comfortable enough with the information to provide a concise and accurate response. All questions must be answered. The video may not exceed more than seven minutes. Once you have recorded the video, posts your recording in the discussion board for others to watch. After posting your video, review three other students videos and analyze their communication style and the information provided. Respond to their posts by addressing the following items. One, provide constructive feedback on what the student did well when communicating with the patient. Two, provide constructive feedback on how the student can improve their communication with the patient. Three, provide constructive feedback on what information the student forgot to provide to the patient. And four, provide constructive feedback on the accuracy of the information provided to the patient. Your responses must be in complete sentences, use proper proper grammar and sentence structure. They must be professional and kind. Posts identified to be unprofessional, mean or inappropriate in any manner will result in a greater zero provided for the entire assignment.
