Plan: Include the community or public health nurse’s role in the development and implementation of the interventions for each nursing diagnosis.
Intervention: Provide details about the intervention (s) and any community stakeholders necessary for carrying out the intervention.
Evaluation: Provide a summary of an evaluation plan that includes goals and criteria to evaluate the effectiveness of the intervention(s).
