Write assessment and care plan based on a case scenario presented in class (Richard Cardinal: Cry from the diary of a Metis Child). Students will choose a point in time in Richard’s life while he was living in foster care to create this plan in relation to. Students will use this space to incorporate their own critical reflection of the case presented in class while completing a written assessment and case plan for intervention using input gathered from the case scenario.
The assessment and case planning assignment will be written in 15-20 pages and will include:
– A family systems overview, such as a genogram, detailing family history, strengths, and areas of growth (2-4 pages)
– Care plan domains (7-10 pages), including an overview of and recommendations towards strengthening Richard’s:
o permanency (physical, legal, relational and cultural)
o identity
o health (physical, mental, spiritual)
o behavioural development
o education
o social/relational activities
o self-care and independence skills
Care plans should assess and address each domain and provide tangible recommendations to strengthen each of these domains over a specific period of time.
– Analysis of intersectionality’s from critically reflexive space (5-10 pages), students will reflect on their experience of creating this plan and reflect on learnings from this course. This is a space to capture the strengths and utilities of creating such a plan as well as to consider the shortcomings of planning using such a tool.
The assessment assignment requires critical reflection of the engagement and assessment process as well as the factors influencing the case. Drafts of the assignment will be required to support student learning and engagement.
This assignment will offer students the opportunity to examine their own practice as well as build upon their documentation, critical reflection, and relational skills. This assignment will be written in 5-6 pages using APA 7 formatting.
