This assignment should be executed with only the LEC scope of practice in mind. If you have additional credentials that expand your scope of practice, please do not include those in your role-play. Points will be deducted for going out of the scope of practice of a lactation educator/counselor. If there is a complex breastfeeding issue, the client should be referred to an IBCLC or other appropriate provider. However, there is so much you can do to support your clients before they are referred out!

• Infant weight pattern
• Client education of signs of milk transfer
• Risks of formula
• Appropriate use of formula
• Reassurance for breastfeeding success
“Kamara is a first-time parent. She comes to you in the outpatient clinic with her daughter, Breanne, who is 5 days old. Breanne was born vaginally at 41.1 weeks. The pregnancy and birth were normal, no complications. Breanne’s birth weight was 7 pounds 11oz (3.49kg). You check her weight today and it is 7 pounds 6oz (3.35kg). This is a 3.9% weight loss. In the last 24 hours, Kamara reports that her daughter had 6 wet diapers and 3 dirty diapers which consists of yellow, seedy poops. Mom states she has pain when her baby latches on and now her nipples are very sore. She gave the baby one bottle of formula last night because the baby was cluster feeding and Kamara couldn’t handle the pain anymore.”
Create a POC for this client. Within that POC, be sure to answer the following (note, this is not all that you will include in your POC!)
a. What would you tell Kamara about giving her baby 1 bottle of formula?
b. What recommendations would you make to Kamara to resolve the issue of painful nipples?
