Assessment: Describe the condition that you will be teaching your patient and/or family.
Outcome: What is the goal for the education being provided? What should they do to avoid a certain condition, or what should they do if they are affected by the condition?
Teaching Plan: This should be the actual educational steps provided to family. What teaching methods would be most effective for patient/family education? Handouts or booklets with pictures? Audio or video?
Evaluation: What criteria will be used to assess the family’s understanding of the education provided? Verbalization? Return skill demonstration?
